Your First Visit
YOUR FIRST VISIT TO OUR OFFICE
After a comprehensive exam and diagnosis of your endodontic condition, Dr. Kreutzer will carefully explain the diagnosis, recommended procedures, alternative treatment options, risks associated with treatment and will answer any questions you may have. Dr. Kreutzer considers your understanding of your treatment a critical aspect of your care.
To help us process your information as smoothly as possible, please bring with you a list of all medications you are currently taking, noting the dosage and timing of administration.
When making your initial appointment please let our office know if you have a history of heart valve dysfunction, artificial joint or cardiac valve replacement, are currently immunosuppressed, or have been instructed by another dentist or physician to take antibiotics prior to dental appointments.
Fees / Financial Information
The fee for your endodontic treatment will be based on the extent of treatment. During your first visit we will discuss the probable number of visits, their length, and the fees involved.
Endodontic fees usually range from $800 to $1,500+ depending on the type of treatment pursued and case complexity. Dr. Kreutzer and his staff strive to deliver the highest quality care available at the most reasonable cost to our patients. Therefore, payment is due at the time service is rendered.
Because of the unique nature of a referral based practice and the specialized kind of cases we treat, we ask for a 25% deposit of the total cost of our services rendered if we are billing dental insurance on your behalf.
As a courtesy to you, we are happy to submit your claim to your dental insurance carrier. Please be sure to provide us the correct information to facilitate our filing of your claim.
Be advised that coverage verification does not guarantee payment by your insurance company. While we try our best to estimate your out-of-pocket expense, actual payment is determined by the insurance company once they process the claim.
